Public Schools Selected To Complete The Survey for the 2022- 2023 School Year

The Student Weight Status Category Reporting System (SWSCR) collects weight status category data (underweight, healthy weight, overweight or obese, based on BMI-for-age percentile) on children and adolescents attending public school in New York State, outside New York City. 50% of all public schools are surveyed each year.

  • Notification Letter to District Superintendents/School Nurses (NYSDOH 6/22)
Identifying school districts selected to report Student Weight Status Category information for the 2021-2022 school year.
  • Districts Selected for 2022-2023 SWSCR Reporting (NYSDOH 6/22)
List of school districts required to report aggregated information from school health certificates/appraisals of students in PreK or K, 1, 3, 5, 7, 9, and 11 in the previous school year (2020-2021). Due to students participating in remote learning and other factors associated with COVID-19, schools may not have been able to collect health certificates from all enrolled students. Please report the weight status category data from the health examination forms you were able to receive during the 2020-21 school year.

BOCES, Charter, Private, and 4201 designated schools are not required to report WSC data.

NYC schools are also exempt from state education law that requires this data to be reported to the NYS Department of Health. The NYC Department of Education monitors student weight status using NYC FITNESSGRAM.
