The National Association of School Nurses White Paper: Translating Strategies into Actions to Improve Care Coordination for Students with Chronic Health Conditions (2018) is the outcome of an April 2018 NASN Round-table in Atlanta

It provides education, resources, and actionable framework to help those who create policy and programs in schools understand the impact on chronic health conditions on student success and how school nurses can be key on-site experts in helping address the challenges students and families face and creating a bridge between the school, home, and medical providers.
