Parental Notification Resources 

To obtain passive parental permission, schools may provide notice to parents via the school newsletter, website, the pre-Kindergarten or Kindergarten registration packet, and/or other reasonable means.

The forms below are for parents who wish to opt out of reporting.

Health Provider Notification Resources

It is recommended, but not required, to follow up with the health care provider if they omit the WSC/BMI from the Health Appraisal.
