NYSDOH Immunization Survey

The NYSDOH Immunization Survey Schedule is Changed for the 2026-2027 School Year.

  • The Survey for Pre-K through 12th Grade opens on October 5, 2026, and closes on November 13, 2026.
  • The Survey for Day Care opens on April 19, 2027, and closes on May 28, 2027.

NYSDOH Immunization Survey - Online School Assessment Survey (OSAS)

All schools must complete the online survey to report the immunization status of students in PreK-12.
Homeschooled students are not to be included on the survey.
Students receiving home instruction from the district are included on the survey.

NYSDOH Immunizations- Child Care Programs, Schools, and Post-Secondary Institutions Page

Schools (K-12) & Child Care Programs and Pre-Kindergarten Immunization Survey- Opens on October 5, 2026, and closes on November 13, 2026.
Who Completes: Pre-Kindergarten programs housed within and administered by a school, Schools (Public, Private, Charter, BOCES, OCFS) that house grades kindergarten through twelfth.

Day Care Immunization Survey- Opens on April 19, 2027, and closes on May 28, 2027.
Who Completes: Small Day Care Centers (not in a personal residence), Standalone private Pre-Kindergarten programs that operate outside of a home, Head Start Programs, Nursery Schools.
