New York State Department of Health Confirms That Childhood Vaccination Policies Remain Unchanged 1/6/26
Press release in response to vaccine changes at the federal level. 

The "PreK-12 School Immunization Survey 2025-2026" is no longer accessible in
HERDS and has been moved to the SMART system.

The mandatory annual NYSDOH PreK-12 Immunization Survey looks different this year! 
This demo from the Bureau of School Immunizations provides information on
how to access and use the survey on its new platform. (approx. 34 mins)

NYSDOH Immunization Survey - Online School Assessment Survey (OSAS)

 
All schools must complete the online survey to report the immunization status of students in PreK-12.
Homeschooled students are not to be included on the survey.
Students receiving home instruction from the district are included on the survey.
The Survey Opened on November 3, 2025

Prek-12th Grade Immunization Survey Is Due Annually on December 12th.

Day Care, Head Start, and Nursery School Immunization Survey is Due Annually on April 15th.
