Health Examination | Forms | Guidelines

The Required Health Examination Form has been updated to align with the amendments to Commissioner's Regulations Sections 135.1 and 135.4 relating to the Athletic Placement Process. The Tanner Sexual Maturity Scale has been removed. Schools must ensure that an assessment of increased risk of SCA for students participating in athletics is completed. Changes are outlined in this APP for Interschool Athletic Programs FAQ. 

All health examinations must be on the Required NYS Health Examination or EHR equivalent form.
At this time schools may continue to accept the previous Required Health Exam Form (2023).
Moving forward schools must transition to the 2026 version and communicate such to parents/guardians and providers.

Health Examination Guidelines for Schools (NYSED 2022)
Provides local educational agencies with information on obtaining student health examinations completed by a private healthcare provider (health certificates) along with a framework for establishing the mandated health appraisal program (health examination completed by the district medical director). It explains the purpose of the health examination requirements and provides guidance for developing effective procedures including planning, implementation of local policies, and follow-up.

Dental Certificates

  • Sample Dental Certificate (NYSED 4/2026)
    This sample fillable form may be used and aligns with the requirement of public schools to request a dental health certificate at the same time a health examination is required. It may be completed by a registered dentist or NYS-registered dental hygienist.
